Laser Nd: Yag
Laser Nd YAG frequency doubled from 1,064 and 532 nm wavelength and the Q-switched
system is indicated for the treatment of tattoos and benign pigmented lesions.
Currently, solid colors are obtained by wavelengths of 585 and 650 nm, which
enable the treatment of vascular lesions, the green ink tattoos (still the only
laser that can remove all colors of tattoos) and injuries pigment without
affecting the vessels, whereupon the residual purple are minimal.
Clinically, the effect of high energy radiation is reflected by a photoacoustic
effect to mechanical shock on the tissues, we see a splash and a snap, and
observing the epidermal detachment on impact of waste on the laser treated area,
which will produce a ash white color.
Uses:
Normally, the tattoo is removed after 1-4 treatment sessions, professional
tattoos require 3 to 10 sessions. Blue ink is black with a wavelength of 1064 nm,
while the red, orange and lilac respond well to the wavelength of 532 nm. The
flesh-colored cosmetic tattoos or tend to become black lipstick to treatment
with Nd: YAG Q-switched. Treatment with Nd: YAG laser with Q-switched 1064 nm
wavelength may clarify or eliminate this hyperpigmentation, however, not always
achieved and is not recommended to treat this type of tattoos.
If someone receives treatment for this type of tattoo, it must deal first with a
small part. If it gets dark, you can try again with the 1064 nm wavelength to
see if disposal occurs before starting the treatment the rest of the lesion.
The cause of darkening of cosmetic tattoos are believed to be due to oxidation
of iron and titanium pigment. It seems to be a conversion of ferric oxide to
ferrous oxide destruction or intense ignition of ferric oxide. Older tattoos
seem to respond more rapidly, possibly because the density of ink these tattoos
tends to decrease with time.
Technique:
The laser beam is guided through an articulated arm. The size of the spots of
application are varied, from 1.5 to 4 mm. Most of the elimination of blue-black
tattoos with 1064 nm is made with a focus size of 3 mm and energies from 5 to 8
J/cm2. With treatment there is an immediate blanching of the tissue. With these
high energy and a small focus, favoring a significant splash of tissue. For this
reason, the treatment is applied through a semipermeable housing as Vigilon or
place a transparent cylinder on the tip of the laser. When using larger sizes of
focus, 3 and 4 mm, there is less tissue splatter.
Melanin and hemoglobin absorb light from 400 to 600 nm wavelength and produces
small dermo-epidermal damage. However, with visible wavelengths beyond 600 nm,
the hemoglobin absorption is minimal and considerably less than that of melanin.
Therefore, the use of a wavelength of 650 nm makes the tattoo pigment to absorb
more photon energy level of the epidermis and upper dermis (amorphous carbon,
graphite, ink and dyes, organometallic), with lower absorption and therefore a
lower injury potential of skin chromophores such as hemoglobin and melanin, an
important factor since it reduces the risk of hypopigmentation.
This wavelength can also be used in the treatment of superficial pigmented skin
lesions and can affect areas of dermis, but with the advantage of not being
absorbed by the blood and hence will be negligible residual purpura. No changes
in the texture of the skin and scars.
For treatments that are recommended wavelength of 2 to 6 J/cm2.
Results:
Immediately after treatment there is a white-ash, by the formation of dermal
vacuoles, probably by vaporization. Subsequently, for 4-6 weeks, there is a
gradual fading. After this treatment can be repeated.
